I have done it... but I have done it on the basis that I am from the South East of England, which isn't strictly true - are people from the regions not listed excluded from taking part?

Also, without trying to sound overly critical, I think you could have done the questions slightly better... for example, 'God' could be ambiguous or could just make it sound like you're only wanting people to answer about Christianity and presumably people's heaven and hell answers will always be the same, although I could be wrong with that.

I know that there are religions which have a heaven-esque afterlife without a hellish equivalent, but I wasn't aware of any having the reverse.

I was going to say that - most people I know seem to believe in "a god", and "a heaven" - but not many of them believe in hell.

Do you believe in God? Sort of. Depends on your definition of God.
Do you believe in a Heaven? Depends on your definition of Heaven. Probably not the usual one, so no.
Do you believe in a Hell? Depends on your definition of Hell. See Heaven.
Do you follow a religion? Nope.
Do you go to a place of worship? Does Celtic Park count? If not, no.
If answered yes, how often do you go? Every home game.

I need more information regarding this.

No. Too many Celticers think it does though.

Depends how you define "worship". I'm fairly sure you could take any supporter of any team and claim their place of worship was their home arena. I don't think it's quite limited to any one team, or even sport.

And I'd place bets that most of those supporters spend more time and effort worshipping their team than their God.

I'd like to nominate The Chequers as my place of worship.
